Accolade’s registered nurses take time to listen and understand your needs — whether it’s a health issue or other life circumstances that may be preventing you from getting the best care for you. They can help you find a specialist, coordinate care, prepare for your appointments, and answer billing questions. All Accolade nurses are supported by a team of pharmacists, medical directors, and claims and benefits specialists.

Behavioral health: Accolade can connect you with behavioral health experts that can help you with a wide range of needs. Accolade can answer your questions and help explain your benefits so you know which programs are free and which may have a fee.

Condition management: Accolade provides extensive support for all acute and chronic conditions. From condition education and finding a great specialist to helping schedule appointments and coordinating your care, Accolade provides support throughout your health care journey. Accolade can work with your providers to help find the right treatment plan for you.

Case management: Members with complex care needs, including high-risk pregnancy, cancer and transplant, will receive specialized support from Accolade-certified case managers. Their goal is to understand your unique situation and create simplicity, making it easier for you to manage your health.

Prescription support: Accolade can help identify cost savings opportunities, manage multiple medications, and help you to understand and follow the recommended course of treatment.

Maternity: Accolade’s maternity management program provides you with personalized educational materials to support your specific needs at any point in your journey — fertility, pregnancy, birth and post-natal care, including lactation consulting. Your Accolade women’s health nurse is available to answer questions and further assist you in preparing for the birth experience and taking care of a new baby, including adding your child to your health plan.

Pre-admission and post-discharge: Accolade provides education and support prior to a hospital stay so that you will know what to expect. Accolade nurses provide follow-up during hospitalization and also reach out after discharge to answer any questions and ensure you understand the next steps.

Treatment decision support: Accolade encourages a shared decision-making model, which means they help you to fully understand your diagnostic and treatment options. This allows you to make a decision that is a good fit for you and your family. Accolade can also help you find a doctor for a second opinion so that you can feel more confident about your decision. Nurse triage: Clinical support from nurses is available 24/7. Accolade nurses can triage and educate you about your symptoms. They can also help you choose the right level of care and explore options such as urgent care, primary care, telemedicine, or the emergency room.

Lifestyle coaching: Accolade can help you create a care plan that is easy to follow and simplify existing health care needs. Alongside the medical care you may be receiving, having a care plan in place may help to prevent or stabilize chronic conditions. Accolade can also help you take steps towards positive behavior changes (stress management, exercise, tobacco cessation, etc.).

Health Net programs offer tools that empower you to make healthy lifestyle decisions for you and your family so you can live a healthier, more productive life.

To access these programs, log in to your Health Net account and select the Wellness Center, or visit the wellness portal.1

Breast Health

Your UC Blue & Gold coverage includes women’s health services, such as mammograms, annual gynecological exams, family planning services, and other care. You can also take advantage of the Athena Breast Health Network, which serves hundreds of thousands of women who receive breast care at the six UC academic health centers and their affiliates. Athena helps women evaluate and manage their breast cancer risk, access resources, and develop a personal plan with their health care provider. When you receive a mammogram, you are also eligible to use Athena’s breast health screening tool, which provides additional information about your risk of developing breast cancer. And if you’re found to be at a high risk, you may receive counseling by phone or in person from an Athena breast health specialist.

Diabetes Prevention

Food Hub®: Explore American Diabetes Association® healthy and budget-friendly recipes, and other information to help manage diabetes.

Omada®: A personalized program designed to help you lose weight, gain energy, and reduce your risk for chronic conditions like type 2 diabetes and heart disease.

The Sharecare Eat Right Now program is a digital weight-loss and diabetes prevention program (DPP), available virtually and in person. The Eat Right Now weight-loss program is available for members who do not qualify for the Omada DPP.

Telephonic Health Coaching Program: One-on-one, personalized support with a Health Coach. Discuss what to expect when trying to make lifestyle changes, explore what motivates you to improve your health, and address specific health behaviors that affect short-term health risks and long-term goals. Digital Health Coaching Program (Lessons): Multiple programs related to stress, smoking cessation, exercise, weight, gaps in care and more! Working on your own, it will take 14-21 days to complete depending on which Lesson you participate in. Learn more about the Digital Health Coaching Program.

Health Assessments

Online RealAge Programs: Interactive way to address and improve risk factors, such as stress, sleep, nutrition and activity. The RealAge Programs present achievable goals and encourage you to take small action steps, establishing tiny habits to help build confidence and lead to goal achievement. Lessons help you prioritize the changes to your lifestyle and offer you the tools and support to make lasting change. Learn more about Online RealAge Programs.

RealAge Test (health assessment): Provides a personalized report of your behavioral and medical health risks. Immediately after taking the online RealAge Test, you will receive a personalized action plan. Take the RealAge Test.

Manage and Store Health Information

Personal Health Profile: Store, maintain, track and manage your health information in one centralized, private and secure location. Take it with you to your preventive care doctor visit.

Pregnancy

Start Smart for Your Baby®: Offers no-cost custom support to a pregnant person and care for a healthy pregnancy and baby. Members receive health education materials such as the Mother’s Guide to Pregnancy and Guide to Life After Delivery, and can request guidance from a care manager throughout their pregnancy.

Tobacco Cessation

Craving to Quit® tobacco cessation program: Talk with a quit coach for encouragement and support and get a personalized plan to quit. The award-winning 21-day program teaches awareness of cravings and habits to help participants quit smoking or vaping. Get started now!
